Report Forms
Monitoring activity and involvement is one way to measure the effectiveness of LEAVE A LEGACY™ Iowa’s ongoing education and public awareness campaign. The completion of report forms helps us to better serve you.

Gift Report Form
The legacy gift report form reports planned gift cultivation and activity to LEAVE A LEGACY™ Iowa, and should be completed by professional advisors. This form indicates contact information and the type and amount of gift.

Professional advisors are encouraged to report planned gift cultivation and activity to LEAVE A LEGACY™ Iowa. Names of donors are NOT requested in order to respect confidentiality.
