As a nonprofit professional/representative, we hope you will review this resolution, and submit your name to signify your support and commitment to the program.

Your personal endorsement will help achieve the goal – to build support for charitable nonprofit organizations and institutions through estate gift pledges.

Simply complete and mail or fax the endorsement form to the LEAVE A LEGACY™ Iowa headquarters in Des Moines, Iowa.

Those who endorse the program will have the option to be listed on the Web site by name and/or title, organization, address, city, state, and zip.
