Partner registrations can be submitted via fax. LEAVE A LEGACY™ Iowa will send an email confirmation along with an invoice attachment for the participation amount you indicate. Your registration will be processed while we wait for your payment at which time your name will be added to the Web site and included in our publications as a Partner.

You can also print and send your registration along with your payment to the address listed on the form.
